📋 Tanggung Jawab Pekerjaan

Assess and evaluate the medical, social, and psychological needs of patients and their families.,Develop and implement person-centred care plans in collaboration with healthcare professionals.,Provide emotional support, education, and guidance to patients and their families.,Coordinate and facilitate access to medical resources, treatments, and services.,Monitor and evaluate the progress of patients and adjust care plans as needed.,Educate and empower patients and their families to actively participate in their care.,Collaborate with healthcare professionals, social services, and community resources to ensure comprehensive care.,Maintain accurate and confidential patient records and documentation.

🎓 Kualifikasi & Syarat

Bachelor's degree in Social Work, Nursing, or a related healthcare field.,Minimum of 5 years of experience in medical social work or a similar role.,Strong understanding of medical terminology and healthcare systems.,Excellent communication and interpersonal skills.,Ability to work independently and as part of a team.,Proficiency in English, both written and spoken.,Knowledge of Indonesian culture and customs is a plus.,Valid certification or license as a Medical Social Worker is preferred.
